Medication Forms

There are two separate forms below.  Complete the appropriate form (OTC or prescription) and return with medication to the school nurse.  Forms are also available in the front office. Forms may also be picked up from the nurse. All medications have to be brought in by the parent. Students are not allowed to bring in the medication.
